What to expect at your visit
Most Farmington visits start and finish inside one appointment. Because the clinic sits on Bourne Circle just off Park Lane, a lot of our Davis County patients come straight from the Station Park side on a lunch break, so we hold the schedule tightly and get x-rays done on site rather than sending you elsewhere and booking a second visit.
Bring your photo ID, insurance card, a list of current medications, and — this is the one people forget — the shoes you actually wear. For the sports and school-age injuries we see a lot of from Davis School District families, the cleat or the running shoe usually explains more than the x-ray does.patient intake forms before arriving.

You’ll find us at 473 W. Bourne Circle, Suite 2, in Farmington, just off Park Lane near the Station Park shopping district and easily reached from I-15 (Exit 325) and the Legacy Parkway. Free surface parking is available directly outside the building, and the entrance and exam rooms are on ground level for easy access. The FrontRunner Farmington Station is only a few minutes away for patients traveling by transit.
